Post Traumatic Stress Disorder (PTSD)

Terminal Conditions (as diagnosed by your medical provider)
Chronic Nonmalignant Pain*

Your process will consist with filling out the online intake form then contact us to schedule your appointment, call 239-268-3636. We will assist to determine your qualification and any required documentation. If the doctor determines you do not qualify there is no charge to you. Upon approval, we will continue to assist you with your application to the State.

Steps to Certify for Medical Marijuana

Start the process by filling out the intake form online; www.MMJConciergeDoctor.com. then call the Center to make your appointment and request your forms@PCCFlorida.com. Arrive 20 minutes early to complete any forms not filled out prior to your appointment.

Please bring with you any records you may have to qualify your condition(s).
You may also request and complete the medical records release form to submit to your physician.
After a PCC Physician determines that your condition meets the Florida Department of Health requirements, the Dr. will make a recommendation and establish a medical care plan on your behalf. PCC recommends a DNA test to determine your best treatment plan.

After your evaluation you will be added to the State of Florida OMMU Registry.
We will assist you with your application to apply for your MMJ ID Card at www.MMURegistry.FLHealth.gov
Your orders to include flower will be submitted for 210 days (7 months).
Processing by the state will take approximately 10-21 business days. You will receive an email for which you may use with your Drivers License to visit any Florida dispensary to begin your treatment.
