You may begin the process by filling out the Online Intake Form at the site www.MMJConciergeDoctor.com and then by calling 239-268-3636.  We will attempt to accommodate your time frame whenever possible.  Once your appointment is set please complete the state required forms below.

• I don't have one of the States qualifying conditions can I still qualify?

YES you may, if your conditions, symptoms, disease is the Same or Like Kind as the State of Florida's qualifiers. Contact PCC to further discuss.

• How do I get my MMJ Card?

You call or email Physicians Cannabis Centers to evaluate and process you through the system as well to educate, recommend and assist you with your needs and desires with your use of medical marijuana and hemp.

• My Card is expired, do I need to see the Doctor?  How do I renew it?

Your Cards renewal is ANNUAL through the State of Florida.  PCC will assist you to complete your renewal, keep an eye on the date located on the lower right hand side of your card.  The state will mail you a new card within a month of your online renewal.  Contact PCC to assist you with the process.

• The Dispensary says my ORDERS are not active, I  must see the Dr.?

YES, that is correct, for you to continue to be Active and to purchase your medicine, the State requires that you come see the Doctor.  The maximum allowed timeframe for any Doctor to provide Orders is 210 days (7 months).

• What do I need to bring to my appointment? 

Bring with you the diagnosis of your condition(s) or fill out the Medical Release Form so we may receive it direct from your Doctor.  If you are having difficulties with Anxiety, Depression, Panic Attacks, we can assist with the an evaluation with the Psychiatrist to determine your qualifications for the use of Medical Marijuana for your condition.

• What Terpenes are best for my Conditions?  What Strains do I use? Which cannabinoids do I need?  What about Sativa, Indica, Hybrid?

To receive the States required forms send an email request to: Forms@PCCFlorida.com, we will email them to you.  Bring in all completed forms, a list of your medication and your medical records if any, with you on the day of your appointment.  If you are unable to complete your forms in advance of your appointment time, please arrive 30 minutes early to complete the forms at the Center so you may see the Doctor, we can assist you with this.  If you are feeling depressed or have anxiety, are in need of diagnoses and wish to speak with a Psychiatrist for evaluation through telehealth please contact Dr. Paul Maione at DrPaul@ptsdevals.com or 954-817-8730 then request that they forward a copy of your results to Eval@PCCFlorida.com.  PCC is not affiliated with Dr. Maione you may speak to any Dr. you would like to.

This is the most important form to fill out before coming to see the Doctor.  The following link will take you to an online form to fill out, no download required, please complete this form first. This is a State required form, patients must read and initial each paragraph, then sign and date.  Transferring patients must do so with every Doctor you transfer to.  Request this form by email to be sent to you Forms@PCCFlorida.com

For those that would like to use medical marijuana to aid in treating anxiety and depression, that have not currently been diagnosed, may fill out this form. Please send an email to request this Forms@PCCFlorida.com.
